Fondazione IRCCS Ca’ Granda Ospedale Maggiore Policlinico (PCL) is one of the largest hospitals in Milano. PCL will participate with the Geriatric Unit (http://www.nitp.org/reparti/g/Geriatria.html) which every year manages about 1000 new referrals among elderly (age > 75 years) in the outpatient clinic and follows up more than 2,500 elderly outpatients. Patients are routinely visited at least once a year through a comprehensive geriatric assessment approach. Information regarding their global health status, their clinical history, and the social environment are collected at each visit. Tests to assess cognitive ability (MMSE), self-sufficiency and autonomy (ADL, IADL), physical ability (Chair Stand test), Muscular strength (Hand-Grip test), the risk of fall (Tinetti scale), Geriatric Depression Scale (GDS) are administered. Medication review is routinely performed.
